| Because
many people do rush into ferret ownership, LW wants to make sure that
the decision
that you have made is the right one, for both you and the ferret.
Please take a
few minutes to complete the application; it is long and comprehensive.
So, if
at any time while filling out this application, you have questions
and/or
concerns, please don’t hesitate to ask!. This application is
the
first step in the screening process of acquiring a ferret from LW. Once
your
application has been received by LW, a representative will contact you
to move
on to the next step. The process will consist of the following steps: 1)
Filling
out
the Placement
Information
Application and signing the Contract and Terms & Conditions; 2)
A
telephone
interview, in which
your
familiarity of ferrets will be discussed, the ferret’s needs,
and
the
commitment that it will take to welcome ferrets into your home; 3)
LW
may require a reference check of family members,
friends, and/or veterinarians to complete the process; 4) Lastly, LW may require an in person interview to complete the process. |

| CONTRACT AND TERMS & CONDITIONS: |
|
Leizel’s Weasels
will not
place a ferret with anyone who: 2) Does not
have a good
understanding of the needs of ferrets and the
responsibility required for good ownership. 3) Does not
have the means
to provide appropriate food, medical care, time
and attention to a ferret. 4) Wishes to acquire a
ferret
for any reason other than to be a cherished family pet. Ferrets will
not be
placed for use in any working purpose, such as hunting, nor will LW
place
ferrets to be sold in pet stores, used as classroom pets, at petting
zoos, etc. 5) Wishes to use a ferret
solely for breeding purposes. Ferrets will only be placed as breeding
stock to
other experienced, responsible breeders. LW is willing to mentor
beginning
breeders on the ethics of responsible breeding. 1)
The
ferret will be handled
in a loving and gentle manner by all members of the new
owner’s
household and
by all guests that visit the new owner’s home.
2) Daily, the ferret will be
provided with attention, companionship, exercise and discipline. 3) For optimal health, the
ferret will be fed a high-quality ferret or kitten food such as:
Totally Ferret,
8 in 1 Ultimate, Path Valley Farms Chicken & Rice, Innova
Feline,
Royal Canin Feline Nutrition Growth 34, etc. 4) The ferret will be given
fresh water each or every other day, preferably from a water bottle. 5) The ferret will be housed
indoors in a clean and healthy environment in a spacious wire cage or
ferret-proofed room. If housed in a wire cage, the new owner will
provide soft
padding on the floors of the cage to eliminate any damage to the
ferret’s feet. 6) The ferret will receive
the proper yearly vaccinations (USDA approved Rabies (Imrab-3) and
Canine
Distemper (Fervac-D or Purevax) unless otherwise determined by a
licensed
veterinarian due to age, illness, or a severe reaction) and yearly ADV
testing
(done by United blood or Avecon saliva). 7) The new owner will
provide
the ferret with timely medical attention and necessary medical care for
the
life of the ferret. The new owner shall not
have the ferret put to sleep rather than treatment due to financial
setback. 8) The new owner will
provide
the ferret with weekly maintenance/grooming to ensure a healthy and
happy pet. 9) The ferret will be
provided with a pelleted wood or paper litter in their litter box (in
and out
of cage). Absolutely no
wood
shavings. 10) The ferret will be
provided with a hammock, sleep sack, old sweatshirt (or equivalent) to
sleep
in. 11) The ferret will be
provided with safe toys to play with. Nothing
with latex, rubber or small parts that can be chewed off. 12) The ferret will receive
supervised playtimes and will not be left unsupervised in the presence
of young
children, other animals or any individual who cannot remove themselves
from the
ferret’s presence. This is for the safety of all parties
involved. 13) All serious health
issues
(such as all forms of cancers, diseases, vaccine reactions, etc.) and
death of
the ferret must be reported to LW within 14 days. 14) The new owner may not
have the ferret descented,
declawed, or have its canines/incisors removed (unless otherwise
determined by
a licensed veterinarian for health reasons). 15) The new owner shall not
harm, neglect, mistreat, abuse or
otherwise treat the ferret in an inhumane way. 16) The new owner shall
provide LW with any changes of information (address, phone number,
etc.) within
14 days. 17) If the new owner wishes
to sell or give away the ferret purchased from LW, cannot spend quality
time
with the ferret, is unable to keep the ferret due to any reason, is
abusing or
mistreating the ferret (by not following the above guidelines), the new
owner
will notify LW immediately. LW may reclaim the ferret and/or refuse the
transfer and/or the sale of the ferret to another party (individual,
shelter,
or breeder). If LW allows the transfer of ownership and/or the sale of
the
ferret and does not reclaim the ferret, LW will be provided with all
known
information about the new owner (name, address, phone number, etc.)
before the
transfer and/or the sale of the ferret is completed. In the case of the
ferret
being transferred and/or sold to a new owner, LW may require a phone
interview
and/or the new owner to go through an application process before the
process is
completed. If the ferret is reclaimed by LW, the new owner must provide
a
record of all vaccinations and negative ADV testing, along with the
ferret’s
complete medical records to LW at the time of surrender. If any of the
above
conditions apply, no
refund (full or
partial) or an exchange of another ferret will be given. 18) LW cannot and does not
make any representations or
guarantees, either expressed or implied, as to the temperament, habits,
or
background of the ferret being sold. Therefore, in consideration of
receiving
the ferret, the new owner hereby releases LW from any and all claims of
liability for actions of the ferret purchased. 19) The new owner (whichever
letter is circled and initialed by LW is the agreement between both
parties): A.
will have the ferret spayed (for
jills/females) at 6-8 months of age or neutered (for hobs/males) at 7-9
months of age or within
_______ of the
date of purchase. The ferret will never
be bred. A copy of proof
from a licensed veterinarian
must be
sent to LW stating that the ferret has been spayed/neutered within
14 days of the procedure. If
the new
owner does not have the ferret spayed/neutered by the agreed time
frame with proof sent to LW,
LW has
full rights to reclaim the ferret. B.
will
use the ferret for breeding. C.
will
be keeping the ferret as a pet and
has already been spayed or neutered. 20) It is the responsibility
of the new owner to complete the kit series of vaccinations. This
includes the
Canine Distemper (Fervac-D or Purevax) vaccine at 11 and 14 weeks of
age; Rabies
(Imrab-3) vaccine at 12 weeks of age and lastly an ADV test (United
blood or
Avecon saliva) at 12 weeks of age. 21) The initials LW shall be
used in front of the ferret’s name on all show entries,
records,
identifying information,
etc. For example, LW’s Bellagio. Furthermore, the breeder
shall
always be
listed as Leizel’s Weasels. 22) If it is determined in
this contract that the ferret shall be used for breeding purposes, then
the
ferret’s name must have the term “bell”
somewhere in
their title. This title
shall be used for all show entries, records, identifying information,
etc. For
example, LW’s Bellarosa or LW’s Annabelle. The new
owner
shall notify LW of the
ferret’s name within 14 days of purchase and shall not
duplicate
a name already
taken. 23) Owning a ferret is a
lifelong commitment and the ferret may live up to 10 or more years. 24) LW reserves the right to
approve or deny any application for ferret ownership. |
